The Challenge
A community round is a day of short, important calls: a visit running late, a change in a care plan, a family member asking how their mother is. When those calls run over personal mobiles, the carer's own number ends up in the hands of patients and relatives, and it stays there long after the carer has moved on.
The provider, meanwhile, can see none of it. A disputed conversation about medication or a missed visit comes down to recollection, because the call happened on a phone the organisation does not control.
The Solution
Each nurse, carer and coordinator carries a business line on their own handset, delivered by eSIM. There is no application to open and nothing to log into — it behaves like an ordinary mobile, which matters for a workforce that is recruited quickly and works alone.
Because each line is an extension of the provider's phone system, a family's call reaches whoever is covering today rather than whoever happened to hand out a number last year. A carer who is mid-visit can let the coordination team pick up, and the caller is handled rather than lost.
